Office Policies

First-Visit Information
To allow us to get acquainted with you and your health issues, we need to get your comprehensive medical and dental history. We provide new patient forms, which must be completed before we see you. We ask that you arrive at least 15 minutes prior to your or your child’s appointment to fill them out. Or, you may download the forms from our website to print and fill out at your convenience. Just remember to bring them with you when you come in.

Appointment Cancellations and “No Show” Policy and Fee
We make every effort to confirm your appointment two days in advance. If you need to cancel or reschedule, you must do it within 48 hours (two business days) or be subject to a $75 cancellation fee. The $75 fee also applies if you fail to show up for your appointment.

Costs
The cost of treatment depends on the type of procedures and complexity of the patient’s needs. Insurance coverage also varies, and depends on your specific plan’s policies and benefits. You will have a chance to discuss costs and payment options before treatment begins.

We accept most insurance plans, and will file the necessary paperwork with your insurance company. If you wish, we can also provide you with a pre-treatment estimate that can help you determine how much will be covered by your plan.

We work hard to make dental care affordable and to ensure that you’re getting the most coverage possible from your insurance company. We also offer credit and financing options for patients without insurance or whose procedures are not covered.

Dental Insurance
We bill all dental insurance programs, including non-managed care, indemnity (traditional), and PPOs. However, we are not part of any managed care network other than some Regence and Blue Cross Blue Shield plans.

We will prepare all necessary forms for your dental benefits, but we remind you that your specific policy is an agreement between you and your insurance policy. Policies can change, and employers can switch providers or levels of coverage, which can affect the amount of your bill.

Please keep in mind that you are responsible for the amount not covered by your insurance. If you have questions about what your responsibility might be, our staff will be happy to submit a pre-treatment estimate to your insurance company to provide an idea of your out-of-pocket costs.

Payment Policy
The patient portion of your bill (the amount not covered by insurance) is due at the time service is performed, unless a payment was made in advance. This will entitle you to an 8 percent discount. We accept cash, checks, and major credit cards (Visa, MasterCard, and American Express), as well as CareCredit financing. To pay with CareCredit, you must apply for it and be approved before treatment begins.

Copies of Your Dental Records and X-rays
If you want copies for yourself or to send to another healthcare provider, we ask that you put the request in writing. Once we receive your written request, we will make every attempt to have the records available within three business days. Depending on the size and format(s) of the records requested, a fee of up to $20.00 may be applied.

Snow and Inclement Weather Policy
If the weather makes it unsafe for patients and staff to travel to our office, we may choose to close the office for part or all of the day. If you cannot make it to your appointment because of inclement weather, or are unsure whether our office will be open, please call as soon as possible.
